Schedule D 

we prepare a Schedule D1 - which is the official IRS continuation sheet for Schedule D.
All the reportable wash sales are properly accounted for in the body of the Schedule, as prescribed by the IRS in the Schedule D instructions, as well as the IRS Pub 550.

For your own records, we include a summary report, that will explain to you the effect of wash sale losses on your tax situation.  It may look something like this:
